EVALI and "popcorn lung" — separating the vaping scares from the facts
Two phrases dominate scary headlines about vaping: "EVALI" and "popcorn lung." Both point to real concerns, but both are also frequently misused in ways that muddle the actual risk. Understanding what each one is — and isn't — helps you judge vaping on the evidence rather than the loudest headline.
What EVALI actually was
EVALI stands for "e-cigarette or vaping product use-associated lung injury," the name given to a serious outbreak of acute lung damage, mostly in the United States in 2019. It caused hundreds of hospitalisations and some deaths, which is why it became headline news. What often gets dropped from the retelling is the cause: investigations by health authorities strongly linked the outbreak to vitamin E acetate, an additive used mainly in illicit, informally-sourced THC (cannabis) vaping products — not to commercial nicotine e-liquids as a class.
Why that distinction matters
This doesn't make nicotine vaping "safe" — it means EVALI, specifically, was largely tied to a contaminant in unregulated black-market products, and cases dropped sharply once that additive was identified and avoided. Using EVALI as shorthand for "vaping destroys your lungs" overstates what the evidence actually established. The honest version is narrower and more useful: certain additives, particularly in illicit products, caused a specific and severe injury.
What "popcorn lung" refers to
"Popcorn lung" is the nickname for bronchiolitis obliterans, a rare and serious condition that scars the smallest airways. The name comes from cases among factory workers exposed to high levels of diacetyl, a buttery flavouring — originally in microwave popcorn plants. Because diacetyl has been detected in some flavoured e-liquids, the concern jumped to vaping.
The nuance the headline drops
Here's the part usually left out: despite years of vaping and testing, there is no established body of confirmed "popcorn lung" cases caused by e-cigarettes. Diacetyl exposure from vaping is also generally far lower than the industrial exposures that caused the original disease, and it has been removed or reduced in many regulated products. So "vaping gives you popcorn lung" is not something the evidence currently supports — even though avoiding diacetyl entirely is still sensible.
Where the real risks actually sit
None of this is an all-clear for vaping. The genuine issues are less cinematic: nicotine is highly addictive, the long-term effects of inhaling flavourings and aerosols over decades simply aren't known yet, and youth uptake is a real public-health concern. For someone using vaping as a step away from smoking, the reasonable framing is that it's likely less harmful than continuing to smoke, but not harmless — and worth eventually stopping too. For someone who doesn't smoke, there's no health reason to start. The scary headlines aren't the strongest argument; the quiet unknowns are.
